Health Room Information
Registration - New and Returning Students
Students must be up-to-date on immunizations while attending La Luz Elementary School.
Please check in with the nurse to make sure all vaccines are up-to-date when you register your student. This is a requirement by the NM State Department of Health.
Returning students -- please notify the nurse of any new immunizations received.
For medical or religious immunization exemptions please have the exemption form approved by the state prior to the beginning of school in the Fall. This form must be completed and approved by the state annually.
If your student has any chronic illnesses that require medical procedures or medication at school, we must have the appropriate forms on file in the health room, signed by the parent and physician by the beginning of the school year. All health forms must be updated each new school year.
- Some examples of medications or procedures that need parent and physician authorization are listed below:
- Asthma inhalers (including students who self-carry) -- Students must have an asthma action plan.
- Epi-Pens or diphenhydramine for allergic reactions -- Students must have an allergy action plan and special diet form for food allergies.
- Medications such as ADD/ADHD medications to be given at school -- Students must have parent/provider authorization.
- Procedures such as urinary catheterizations or g-tube feedings -- Student must have parent/provider authorization.
- Students with seizures who need emergency medication at school -- Student must have seizure medical management plan.
- Diabetic students will need a diabetes medical management plan from the provider.

Health Screenings
The New Mexico Department of Health provides guidelines in the State Health Manual for routine health screenings of students who are new to the school district and who are in specified grades (the specified grades are subject to change by the Department of Health). The health screenings include:
- Vision Hearing
- Dental
- Height
- Weight
Students being tested for special education will be screened.
Screenings may also be requested by teachers and/or parents. Health screenings are an important assessment in detecting barriers to learning or accessing the learning environment and promoting early interventions. Health screenings are not diagnostic, if a student fails a screening, a referral will be made to your healthcare provider.
If you have any questions regarding health screenings, please contact the school nurse.
If you would not like your child to have health screenings, this request must be submitted in writing to the school nurse annually.
Head Lice
Dear La Luz Parents and Staff,
Per APS Policy we do not send letters out to the entire school if a student or students have head lice. We will send a letter out to the parents in a classroom if three to four students have had lice in that classroom (with the approval of our principal). However, if your child has lice, it is helpful if you let his/her friends' parents know! If you call and notify the health room, I will contact teachers who will then notify me of students "scratching!"

Note: Do not use a combination shampoo/conditioner, or conditioner before using lice medicine. Do not re-wash the hair for 1–2 days after the lice medicine is removed. These measures may reduce the effectiveness of the lice medications.
